- Driver’s license
- College ID with photo
- Workplace ID with photo
- Health Insurance Card (we accept many different insurance plans)
- Co-payment

If you need to cancel or reschedule, please provide 24 hour notice. Failure to do so may result in a fee assessed.
Minors under 18 must be accompanied by their parent or guardian to sign paperwork.
